Deep TMS for OCD
Standard TMS is FDA-cleared for depression and OCD — but treating OCD specifically requires different equipment and a different protocol. Our Deep TMS system uses a specialized coil cleared by the FDA specifically for OCD, targeting brain circuits involved in obsessive-compulsive symptoms that aren't the same ones involved in depression.
We've treated numerous patients with treatment-resistant OCD, and our protocol includes the individualized provocation procedure used in the original clinical trials — a step that's sometimes skipped elsewhere, which may be why some patients tell us TMS didn't work for them before.

Why OCD medications sometimes don't work
OCD often needs a different approach than depression, even when the same medications are used:
- Higher doses. OCD typically needs higher SSRI doses than depression does.
- Longer trials. A medication may need several months at an adequate dose before you can judge whether it's working.
- Therapy alongside medication. Medication often works best when combined with exposure-based therapy.
- A clear diagnosis. Related conditions, such as depression or tic disorders, can change which treatments are the best fit.
What a second opinion at Houston Mind & Brain looks like
Our psychiatric providers start with a detailed review of every medication you've tried: the dose, how long you took it, and what happened. We reserve an hour for your consultation so there's time to do that properly.
From there, we look for gaps. We often see patients who have tried several SSRIs but were never offered clomipramine (Anafranil), an older medication that is FDA-approved for OCD. It requires closer monitoring than an SSRI and isn't right for everyone, but for some people it's an option worth discussing.
Your plan may include adjusting doses, trying a different medication, adding an augmenting medication, or combining medication with Deep TMS for OCD, which is FDA-cleared for OCD. If you already have a therapist or prescriber you trust, we're glad to coordinate with them.

Why didn't TMS work for me before?
This is common, and usually comes down to one of two things. First, OCD requires a different type of TMS than depression does — a specialized coil with its own FDA clearance — and not every provider has that specific equipment. Second, the original clinical trials that proved this treatment works included an individualized provocation procedure as part of the protocol, and that step is sometimes left out elsewhere. If you tried TMS without one or both of these pieces in place, it's worth a fresh evaluation before ruling it out.

Will my insurance cover TMS?
Many commercial plans cover standard TMS when medical-necessity criteria are met. Accelerated/fast-track TMS is often self-pay. We verify benefits and submit prior authorization on your behalf.
What if I’m out of network?
This is uncommon because we are credentialed with most plans. If needed, we can provide a superbill for out-of-network reimbursement.
