In-Network with Major Plans


Aetna, Cigna, Optum, TRICARE, and Medicare. We handle prior authorization and claims submission on your behalf.


Medicare accepted


We bill Medicare for covered services, including standard TMS and Spravato when clinically appropriate. Medicaid is not accepted.


Out-of-network is rare


Because we are credentialed with most plans, true out-of-network situations are uncommon. If needed, we can provide a superbill for reimbursement.

After verifying your benefits, we email you a written estimate showing your expected out-of-pocket responsibility - before treatment begins.

Spravato (esketamine)

Commonly covered by insurance when clinical criteria are met. We are REMS-certified and assist with benefits verification and prior authorization. Self-pay Spravato is rarely offered - contact us for your specific situation.

Administrative fees

Emergency refill requests $75 (refills must be placed ≥72 hours in advance). Forms & letters: starting at $50; final fee depends on complexity.

Who Qualifies

Sliding scale pricing may be available for uninsured patients or those with commercial insurance plans that have a high individual deductible (≥ $5,000) that applies to TMS. The sliding scale applies only to self-pay balances.


Who Does Not Qualify

Sliding scale pricing applies only to TMS treatment and does not extend to provider visits, ketamine services, or psychotherapy.

Eligibility varies, including for certain insured and Medicare patients.


Verification Required

Verification required: Income and/or insurance verification is required before enrollment. Sliding-scale discounts cannot be applied retroactively. Decisions are confidential and returned in about 3 business days.

Accelerated TMS protocols

Accelerated & Specialized TMS

Accelerated protocols are not eligible for the sliding scale and are commonly self-pay. Payment is due at time of service.

SWIFT - 38 sessions

Payment Policies

A few things to know before you start.

Payment is generally expected at time of service. For TMS courses, payment plans can be arranged on a case-by-case basis - contact billing to discuss. Payment plans are not offered for routine medication management.

We accept card and electronic payments. We do not accept cash.

All prices listed are representative ranges. Final charges depend on clinical details and payer rules. Written estimates supersede any web-published figures.

All Sliding-scale discounts require income/insurance verification before enrollment and cannot be applied retroactively.

Medicare: some services must be billed to Medicare for Medicare beneficiaries rather than accepted as self-pay. We confirm billing rules and provide a written estimate before treatment.

Frequently Asked Questions

  • Does my insurance cover TMS?

    Most major commercial plans, Medicare, and TRICARE cover standard TMS when clinical criteria are met. We handle the prior authorization process on your behalf and will tell you what's covered, and what isn't before you begin treatment.

  • What if I have a high deductible?

    If your commercial plan has an individual deductible of $5,000 or more that applies to TMS, you may qualify for our sliding-scale program. We verify your benefits and deductible status as part of the intake process.

  • Do you accept Medicaid?

    No. We do not accept Medicaid. We do accept Medicare, and we bill Medicare when appropriate.


  • Can I set up a payment plan?

    Yes, for TMS courses we can arrange payment plans on a case-by-case basis (e.g., deposit + installments, or split payments over the treatment period). Payment plans are not available for routine medication management - payment is expected at time of service. Ask about a payment plan when you request your written estimate; terms are confirmed in writing.

  • Is the sliding scale confidential?

    Yes. All sliding-scale applications and income verification documents are handled confidentially. Decisions are returned in approximately 3 business days.

  • How does Spravato billing work?

    Spravato (esketamine) billing includes two separate components: the medication and the in-clinic observation time.


    The medication cost may be billed through either your pharmacy benefit or medical benefit, depending on your insurance plan. For eligible patients with commercial insurance, manufacturer savings programs often reduce the cost of the medication to as little as $10 per treatment.

    The observation visit (required monitoring time in the clinic) is billed separately as a medical service. Your cost for this portion depends on your plan and may include a copay, deductible, or coinsurance.


    For patients with commercial insurance, programs such as Spravato WithMe may provide additional reimbursement for observation costs (up to program limits).

    This is uncommon because we are credentialed with most plans. If needed, we can provide a superbill for out-of-network reimbursement.

  • How do I get a written estimate?

  • Can I get sliding scale pricing for TMS?

    At this time, sliding scale pricing is not available for Medicare patients or for accelerated TMS protocols.
