Insurance & self-pay: Pricing may be determined by your insurance coverage or by self-pay. Benefits, prior authorization requirements, and out-of-pocket costs vary by plan and are confirmed individually.
Estimates: Upon request, we may provide written estimates for eligible services before you start care. Estimates are not guarantees and may change based on medical necessity, documentation requirements, payer rules, and other factors.
Changes: We may change service offerings, pricing, and availability at any time.

Appointment requests; eligibility; communications

Governing law; severability; dispute resolution; venue; class action waiver
These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles.
Any controversy or claim arising out of or relating to these Terms or the Website shall be resolved in the state or federal courts located in Harris County, Texas. You and Houston Mind & Brain agree to bring claims only in your individual capacity and not as a plaintiff or class member in any purported class or representative proceeding.
If any provision of these Terms is held invalid or unenforceable, the remaining provisions remain in effect. Our failure to enforce any provision is not a waiver.
These Terms constitute the entire agreement between you and Houston Mind & Brain regarding the Website and supersede prior Website terms.
